How do I get custom orthotics at ProActive?

  • Book with a ProActive Chiropractor

  • Check with your insurance plan to see if they require a doctor’s note for coverage

  • Bring your MD prescription (if required) into your orthotics visit

  • Expect the average time to receive your orthotics to be 3 weeks (dependent on time of year)

You will receive a package eligible for your insurance that includes your Receipt of Payment, a Dispense Form indicating date of exam, casting and dispense, biomechanical assessment and exam findings, as well as Diagnosis. It will also include a Manufacturing Document outlining materials used and method of manufacture from Premiere Orthotic Lab.

2nd pair of Custom Foot Orthotics — $300* Ordered at the same time as the first pair.
