How do I get custom orthotics at ProActive?
Book with a ProActive Chiropractor
Check with your insurance plan to see if they require a doctor’s note for coverage
Bring your MD prescription (if required) into your orthotics visit
Expect the average time to receive your orthotics to be 3 weeks (dependent on time of year)
You will receive a package eligible for your insurance that includes your Receipt of Payment, a Dispense Form indicating date of exam, casting and dispense, biomechanical assessment and exam findings, as well as Diagnosis. It will also include a Manufacturing Document outlining materials used and method of manufacture from Premiere Orthotic Lab.
